In Acrobat Pro I can select the Advanced//Forms//Page Templates menu option
and set a "Name " to the current page.
This process adds a "Names " dictionary containing a "Pages " dictionary
coDesigner created templates are subjected to other rules the iText only
follows with filling fields. If you flatten the template and loose all the
