     The usual cause of this is an application that tries to
optimizing the final PDF by sharing data structures between
pages. If the produced PDF isn't going to be manipulated - that's
perfectly valid. But as you found, it can also cause issues when
manipulating it.

     The only two programs that I know of that can do this are
Adobe Acrobat Standard/Professional, and PDF Enhancer
(http://www.apago.com).
