Generated PDF intermittently crashes browser and then starts working

We're experiencing a bug where our generated PDF crashes the browser (both IE6 and Firefox 1.5).  This only occurs intermittently and only initially.  After a while, the problem goes away and doesn't seem to re-occur.

We were using Acrobat Reader 7 and iText 1.4.  We've seen it on WL 8.1 SP5, Solaris, https.  One of the other devs has seen it locally, Windows 2000, Resin 3.0.19 but I haven't had any problems.

Initially though we corrupted the document but it seems to be fine.

I've just upgraded to iText 1.4.2 and haven't seen the problem again but given the intermittency, I'm not fully confident that this has gone away.

I'm wondering if anyone else has encountered similar problems and how they can be dealt with?
