What I can't work out is
 do I create the FormXObject (somehow) and set that as the /N value
OR
 can I just create the text.Image and set that as the /N Value

Also - Is it okay to leave the PopUp Annot off or does it have to be there?

Create a stamp and put the image in the /N appearance.
