You are overlooking it.
There's an example on how to retrieve the Bookmarks as
an XML file in chapter 2 of the book 'iText in Action'.
That's probably the easiest way to deal with bookmarks
from an existing PDF.

I have put the examples from chapter 1 to 3 online:

Look for HelloWorldBookmarks with Ctrl+F.
