I need to put headings at the top of each page if the information
continues from the previous page, with a maximum of 3 levels of
sub-headings.  For example:



1.0    Bulid environment (continued)

a.       node name (continued)

- software environment (continued)



I have tried a few approaches, but they are all very complicated.  Would
columntext or a table help me?  The biggest challenge I had was to find
out wat the indentation (x-value) of the first line of text that WILL be
printed on the top of the page.
