That was it -- just a quick "very likely" test before we dive into the real troubleshooting. ;)
The next step would be to check the logs for JK2 and Apache, and see what they're doing with the request for "/".
Also, check Tomcat's log and see whether the web.xml is throwing an error processing web.xml. I believe Tomcat will disable the entire context if that happens but I'm not certain.
You mentioned that you're testing the app under NT, instead of under the Linux server where this was deployed. If you could test the actual Linux host, that would yield more informative results.
